Patellar tendon tendinopathies have always been an important and recurring issue in many sports. Even in the case of the patellar tendon, the first hypothesis of inflammatory pathogenesis has been replaced by the degenerative theory, and the term “tendinitis” was soon replaced with that of “tendinopathy.” In addition to the hypothesis inflammatory, over the years, several theories have been formulated in the context of its pathogenesis. Conservative treatment proposed by various authors and that is possible to find retrievable in the literature ranges from functional rest, the use of nonsteroidal anti-inflammatory drugs, modification of training techniques, stretching, and eccentric training. However, a not insignificant number of athletes demonstrate nonresponsiveness in respect of conservative therapies, and in front of a long period of persistence of symptoms, a surgical solution must be considered. [17]Ultrasound offers several advantages; it is time and cost-saving, non-invasive, repeatable, and accurate, and provides a dynamic image of the knee structures. [2]Both ultrasound and magnetic resonance imaging (MRI) can be used to detect abnormalities in the patellar tendon itself. Imaging can also be used to guide clinicians as to the severity of the pathology. [18] Capasso G, Testa V, Maffulli N, Bifulco G. [25] [26] [2]Eccentric training has been shown to have equally efficacious resultsas surgical treatment. It is recommended that eccentric training should be tried for twelve weeks before offering surgical treatment. [27]According to Rodriguez-Merchan eccentric training appears to be the treatment of choice for patients suffering from patellar tendinopathy. [28]
